Key Request e-Form

Complete the following e-form and submit it electronically. Chemistry personnel will then prepare the typed University key form and gather the required signatures for you. The signed and approved copy of the typed form will be available for pickup in Gilbert Hall 153 (the Chemistry main office) within about 48 hours.  Take the approved typed form to the University Key Shop at 510 SW 15th Street . There is a $10 deposit required for each key; this is refundable upon return of key to the University Key Shop. You are required to pay the deposit when you get your keys. A photo ID is required and the official University approved typed form must be signed by the Department Chair. If you need to return a key for any reason, please call the University Key Shop at 737-3565 between 8:00 am and 4:30 pm to arrange a time to make the change. If you are requesting keys to rooms in more than one building, please submit a separate e-form for each building.

Once this e-form is received, staff will prepare the University key request form and obtain the proper authorization from the room & building manager and the Department Chair.  On average, it can take about 48 hours to process your request.  The completed University key request forms are made available to you in Gilbert Hall 153 and you will need to take this form to the University key shop with your University ID and your money.
